A poster presented at the 2023 annual meeting of the American Academy of Ophthalmology showed that when adding goniotomy using the iAccess Precision Blade (Glaukos) to trabecular microbypass using iStent inject W (Glaukos) during phacoemulsification, more patients achieved ≥20% reduction in intraocular pressure than with trabecular microbypass and phacoemulsification alone.
In the prospective, nonrandomized, unmasked, 2-surgeon study, patients underwent phacoemulsification and trabecular microbypass without concurrent goniotomy (group A; 48 eyes) or with concurrent goniotomy (group B; 104 eyes). Mean baseline IOP in groups A and B was 14.88 mmHg ±4.08 and 14.53 ±3.64 mmHg, respectively. Both groups A and B exhibited significant (P<.05) reductions in IOP (13.04 mmHg and 12.19 mmHg, respectively) by month 6. In group A, 34.8% of patients achieved an IOP reduction of ≥20%, and in group B, 44.7% of patients achieved an IOP reduction of ≥20%. Neither group exhibited any severe adverse events with trabecular microbypass or goniotomy.
